Who We Are

VISION: Hospice of Martha’s Vineyard will be the Island’s resource for end of life care available to our entire community. MISSION: To provide dignified, compassionate and individualized end-of-life and bereavement care for our Island community.

Volunteers are an important component of the Hospice team and can help out with patient care, development and fundraising, or administrative work. 

What We Do

Hospice and Palliative Care of Martha's Vineyard provides Medicare-certified and CHAP-accredited hospice care as well as supportive palliative services to our patients and their families, as well as and grief and bereavement services. We provide services to those facing life limiting, complex, serious illness with poor prognosis. We offer guidance on advanced care planning, with a focus on early and late goals of care, management of symptoms at home, care coordination and individualized symptom management throughout the transitional care journey.
